A. OVERVIEW

1. Description and Purpose This policy establishes policies and procedures for employee health and insurance coverage for the City of Saint Paul (City).

2. Applicability This policy applies to all employees of the City.

3. Failure to Comply Failure to comply with this policy may result in disciplinary action in accordance with any and all disciplinary policies of the City.

B. POLICY AND PROCEDURE

1. The City will provide, as long as financially feasible and a plan is available from a provider, a comprehensive group health program for eligible employees. Full details about this program will be given to employees after sixty (60) days of employment with the City.

2. The City will also provide, as long as financially feasible and a plan is available from a provider, a life insurance program for eligible employees.

3. Eligible Employees

a. Regular employees whose work schedule is at least thirty (30) hours per week are eligible to enroll in health insurance coverage along with their qualifying dependents.

b. Regular employees whose work schedule is at least thirty (30) hours per week that enroll in health insurance coverage are eligible to enroll in dental insurance coverage along with their qualifying dependents.

c. Regular employees whose work schedule is at least thirty (30) hours per week are eligible to enroll in life insurance coverage.

4. Employer Contributions

a. Eligible employees will receive an employer contribution equal to:

i. Health coverage is paid at 100%

ii. Dental coverage is paid at 0%

iii. Life insurance coverage is paid at 100%

b. The City due to finance constraints, budget approval, and provider coverage may change the employer contributions at any time subject to approval by the City Council.

5. Employee Premiums

a. Eligible employees are required to contribute the following:

i. 0% for health coverage

ii. 100 % for dental coverage

iii. 0% for life insurance coverage

b. The City due to finance constraints, budget approval, and provider coverage may change the employee premiums at any time subject to approval by the City Council.

6. Qualifying Dependents Premiums

a. Eligible employees are required to contribute the following for qualifying dependent coverage:

i. 100% for health coverage

ii. 100 % for dental coverage

7. Plan Enrollment and Start of Coverage

a. Eligible employees are may enroll in the City’s health insurance program the first day of the month following completion of sixty (60) days of employment. However, health insurance coverage will not take effect until after ninety (90) days of employment.

b. Eligible employees may enroll in the City’s life insurance program upon completion of ninety (90) days of employment.

8. Opt Out Option

a. The City shall pay eligible employees sixty dollars ($60.00) per pay period or one hundred twenty dollars ($120.00) per month to opt out of health insurance coverage.

b. The City due to finance constraints, budget approval, and provider coverage may change the employee opt out payment at any time subject to approval by the City Council.

c. Eligible employees that choose to opt out of health insurance coverage shall complete a Waiver of Coverage form and turn it into the payroll clerk.

d. Eligible employees can only opt out of health insurance if they have can provide evidence of other health insurance coverage.

9. Any increases in premium costs over prior year amounts which are attributable to medical inflation or other factors apart from claims experience and so identified by the insurer will be borne by the City.
